Approved Private Agency

Private agencies seeking approval from the Office of Special Education (OSE) to provide special education services to students with disabilities must complete and submit the following:

  1. Application Form
  2. General Assurance Statement
  3. Staff Certification Form, including copies of teacher certifications and licenses for related service providers
  4. Conduct Facilities Self Study with Standards to Approve Private Education Agencies form and submit dates of safety inspections/occupancy permit

Once all requested information is received, an OSE representative will schedule an onsite inspection visit. During the visit, the representative reviews the agency’s facilities, policies, supplies/equipment, and required staff. The representative reviews the Standards to Approve Private Education Agencies form.

OSE will then grant the status of Approved without Conditions, Approved with Conditions, or Not Approved. A follow-up onsite visit may be scheduled within one year of an agency’s initial approval. Approved Private Agencies must apply yearly for renewal by submitting a signed Assurance Statement and Staff Certification form with updated information by June 15th. Random onsite visits may be performed at any time. Annual notification will be sent indicating approval status for each school year.
