The Missouri Commission for the Deaf and Hard of Hearing (MCDHH) is accepting nominees for the Lifetime Achievement Award which recognizes Missourians who are deaf or hard of hearing or other individuals for outstanding service over a significant portion of their lifetime.  The award will be presented at the Missouri Interpreters Conference, October 12, 2012, at Tan Tar A Resort, Osage Beach, Missouri.

Any Missouri resident may nominate a person to receive an MCDHH Lifetime Achievement Award by sending the name of the nominee, current contact information for the nominee (mailing address, telephone number, and e-mail address if known), a statement of why they think the nominee deserves the award, and, if possible, a brief resume or biographical sketch - and photograph (headshot) of the nominee.

No person shall be eligible to receive an MCDHH Lifetime Achievement Award more than once during their lifetime.  Past award recipients have included interpreters, educators, and service providers, as well as deaf and hard of hearing individuals.

Nominations are due no later than September 14, 2012, and should be submitted to the Executive Director of the Missouri Commission for the Deaf and Hard of Hearing.  Nominations may be submitted by e-mail (mcdhh@mcdhh.mo.gov), fax (573-526-5209), or mail to 1500 Southridge Drive, Suite 201, Jefferson City, MO 65109.
