Subject: ACTION REQUIRED: End of Year Data Collection for 2012-13

The Individuals with Disabilities Education Act of 2004 requires the state to collect various data from districts. Most of this data is collected through MOSIS/Core Data with a few additional collections gathered through other reporting processes. One collection outside of MOSIS/Core Data is for students who are parentally-placed in private schools.
DISTRICTS MUST REPORT THE FOLLOWING INFORMATION FOR PARENTALLY-PLACED STUDENTS ATTENDING A PRIVATE SCHOOL WITHIN THEIR JURISDICTION.
1. The number of students WITH INITIAL EVALUATIONS conducted during the 2012-13 school year.
2. The number of those evaluated that were found eligible.
3. The number of those found eligible that received special education or related services from your district.
Note: This is NOT a total count of parentally-placed private school children served by your district.

All districts must submit this information even if all of the responses are zero. All districts must submit the form by June 30, 2013.
