A Road Map to the Future: Person-Centered Planning will be held on Thursday, March 15, 2012, from 9:00 a.m. to 3:30 p.m. at the Stoney Creek Inn and Conference Center in Columbia, MO. "Person-centered planning" is a transition tool used to identify vocational, social, home, and community targets to be achieved by the time the student leaves school. "Person-centered planning" compliments the IEP process and can be used as a tool to inform IEP team decisions. "Person-centered planning" can be used to identify educational activities which integrate functional student needs with academic content. "Person-centered planning" combines features of Making Action Plans (MAPs), Group Action Planning (GAP), Planning Alternative Tomorrows with Hope (PATH), and Choosing Outcomes and Accommodations for Children (COACH) to provide a means for coordinating educational planning.
