This resource toolkit assists in transition planning for youth with disabilities transitioning from high school to live and work more independently in their homes and communities. This resource aims to enhance joint service delivery and collaboration between Centers for Independent Living (CIL), Local Education Agencies (LEA), and Vocational Rehabilitation (VR).
Three main sections walk users through a comprehensive transition planning process: exploration, empowerment, and employment and training. In addition, the toolkit also includes information on disability-related laws, the team approach and other helpful resources including office listing information and helpful links.
