Attention: Opportunity to share the transition practices and programs you have for students!

As announced at this summer's Transition Institute, the Missouri Department of Elementary and Secondary Education and the Transition Coalition are pleased to announce the opportunity to have your transition practices, materials, programs, and successes shared with others in Missouri.

Applications are due October 1, 2008.  You may access the application at http://missouritransition.org/moodle/index.php then choose Missouri Community of Practice and Upcoming Opportunities.  You will need to log into the site or create an account if you have not done so.  You may also access the application on the DESE website at http://dese.mo.gov/divspeced/documents/ModelofSuc.pdf.

Applications will be reviewed by a team of transition experts, and staff at the Transition Coalition will work with approved models to finalize descriptions and materials to post on http://missouritransition.org/moodle/index.php as well as http://transitioncoalition.org/transition/index.php.  Selected models will also be highlighted at face-to-face events (conferences and training) in Missouri and possibly nationally.  Missouri RPDCs and Transition Liaisons will be informed of the models and may use them as examples in training sessions they provide.

To see examples of other Models of Success, go to http://transitioncoalition.org/transition/index.php, choose Collaboration, then choose Models of Success.

Send us your successful and creative practices and programs, and we will help you share them!
