NOTE: The ECSE Bubble Sheets were due June 6, 2006. If these have not been
sent, please do so ASAP! All Bubble Sheets should be received no later than
June 30, 2006.
If you are waiting to mail because you have not received a header sheet,
please go ahead and mail them.
If you are mailing more than one school district, please clip together with
the name of the school district and they will be matched with a header
sheet.
If you have mailed bubble sheets and receive an additional packet, please
disregard. The last round of requests has been mailed as of this date.
If you have NOT REQUESTED bubble sheets, please send the number of packets
you need, mailing address, school district number, phone number, and contact
name to lori.conner@dese.mo.gov. |
