Subject:  Severe Disabilities Services Fund (SDSF) Eligibility Determination

This is a reminder that the eligibility determination part of the SDSF fund is due by June 1, 2005.  The SDSF fund is for those students who meet the cognitive level required for acceptance at State Schools for the Severely Handicapped and do not require a separate school placement.  Services for these students must be implemented in district operated schools or programs. The SDSF fund has a two step process; the first step is eligibility determination, the second is an application request for reimbursement.

 

Step One: Student Eligibility Determination

Deadline: June 1, 2005

 

To qualify a student for reimbursement from the SDSF for the 2004-05 school year, a completed application form, along with a copy of the child’s current IEP and current Evaluation Report, must be sent to the Division of Special Education - Funds Management, Attn: Shelley Witherbee, P.O. Box 480, Jefferson City, Missouri, 65102-0480 and MUST BE POSTMARKED BY JUNE 1, 2005.  The application (Appendix B) needed for this step is located on the last page of the Severe Disabilities Services Fund Manual which is posted on the website listed below.

 

Step Two: Application Request for Reimbursement

Deadline: November 15, 2005

 

The district will need to submit an application request for reimbursement for the students determined eligible for the SDSF fund.  The Excess Cost Application for Severe Disabilities Services Fund is currently being revised and will be available on our website at a later date.
