Updating the name on Certificate

You will need to submit the following items to our office:
 

  • Completed Application for Duplicate/Updated Missouri Teacher Certificate;
     

  • $25 check or money order made payable to "Treasurer, State of Missouri;" and
     

  • Documents that establish the name change (Marriage License, Driver's License, or Social Security card).
     

  • If you are changing name/address only, there is no fee.
