Payment

1. For MOAPS submissions, you may be required to pay at the time of fingerprinting.
2. If payment is to be made by the agency or organization, payment arrangements may be made with IBT.
3. Personal checks, money orders, and credit cards are accepted.

FINGERPRINT INFORMATION FORM

1. Call (866) 522-7067 or go to http://www.L1id.com to schedule an appointment.
2. The applicant provides certain identifying information, including:

School County-District Code (This six digit number will ensure that DESE returns the results to the correct district or college.  If you are not currently employed with a school district, use 999999 for the county-district code and the results will be returned directly to you.)

3. The applicant will choose from available appointment times and locations.
4. At the scheduled appointment, identification of the applicant will be verified by using a photo ID, such as a driver's license, passport, military ID, etc.
5. Applicant must be ready to pay the appropriate fee at the time of fingerprinting unless other arrangements have been made by the hiring/licensing agency.
6. Applicant will receive a receipt that will include the transaction identification number.  This number can be used to track the transaction through the entire process.

Average time to complete the fingerprinting process is ten (10) minutes.  Results should be returned in two (2) weeks.


Request for Child Abuse or Neglect/Criminal Record

REQUEST FOR CHILD ABUSE OR NEGLECT/CRIMINAL RECORD FORM
 

In order to have a Missouri Highway Patrol, Sexual Offender Registry Check and Child Abuse Check completed, be sure to mark box (1) Name Search (criminal record, offender registry, and child abuse search) and send one (1) original and one (1) copy of each form to DESE (criminal record/offender registry and child abuse search). There is a $9.00 charge per individual. Only a school check or money order, if sent by the individual, made payable to “Treasurer, State of Missouri" will be accepted. This fee is the amount that the Highway Patrol charges to process the background checks.

The individual applying for a background check must complete the form and must print CLEARLY. All boxes should be completed including applicant’s full name, date of birth, social security number, and sex. Without this information, the form will be returned to the requesting school district. The address at the bottom of the form should be completed with the name of the requesting school district.

The Highway Patrol and Sexual Offender Registry Checks will be completed and returned to the school district within 1-2 weeks.

The Child Abuse Check will be completed by the Division of Family Services and will be returned directly to the requesting school district within 2-3 weeks.

Requests received with through e-mail or by fax will NOT be accepted.

While you may make as many copies of the form as needed, original signatures are required. Only the FRONT page of the form is required to be received by our office.
