Subject: UPDATES to Model Forms: IEP and Review of Existing Data (RED)

The Department has updated some of the model forms.  If your district uses the Department's model IEP and RED form, please begin to use the updated version immediately.  If your district uses a different IEP and RED form, please review that form to ensure that it is in compliance with state and federal requirements.  Contact the Compliance Section in the Office of Special Education at 573-751-0699 if you have questions.

The Department has updated the model forms as follows:

UPDATE to the IEP form:

The World Class Instructional Design and Assessment ACCESS (WIDA-ACCESS) test is a state-wide assessment given to students who are English Language Learners and must be considered in the IEP process for students with disabilities who are also English Language Learners.  Page 3 of the Department's model IEP form has been updated to include this consideration for the question "Does the student have limited English proficiency?" as well as consideration for "State Assessments."  Form D has been updated to include the accommodation information for the WIDA-ACCESS.  Both of these forms have also been updated to include the correct terminology for the "Grade-Level Assessments" for grades 3-8 for the MAP.  Page 5 has been updated to include the option of Curb to Curb as an accommodation under Transportation as a Related Service.  Districts need to be aware of the difference between "door to door" and "curb to curb" when choosing an accommodation.  Grammatical updates have been made on Form E.  http://dese.mo.gov/divspeced/Compliance/IEP/Index.html

UPDATE to Model RED form: 

Page 5 of the Department's model Review of Existing Data (RED) form has been updated to include the same language that can be found in Federal Regulation 300.305 (d)(2) and the Missouri State Plan for Special Education (Regulation III page 35) for the reevaluation decision of "No additional data needed."  This updated form shows that parents have the right to request additional assessments to determine whether the child continues to be a child with a disability and to determine the child's educational needs.  http://dese.mo.gov/divspeced/Compliance/MOSample_Forms/Index.html
