Every district that received Title I.A funds for the 2010-2011 school year is required to submit a Title I.A End-Of-Year Report.  Our office is required to federally report the data contained on this form.  Please complete the form and return to our office by September 1, 2011. 

Complete only those sections of the form that pertain to your district.  If the information contained in the Title I.A End-Of-Year Report does not pertain to your district, please complete only page one and fax to our office.  The Title I.A End-Of-Year Report is available at http://dese.mo.gov/divimprove/fedprog/grantmgmnt/publication_and_forms.html.
