Spring 2012 District Self-Monitoring Quality Assurance
As part of the Department’s Quality Assurance (QA) process this year, we are continuing the district QA self-monitoring tool for the spring of 2012. This form is designed to be used by District Test Coordinators (DTCs) as part of their regular supervision process throughout the assessment window. DTCs should complete this form twice during the Spring 2012 assessment window – once for one Grade-Level classroom and once for one End-of-Course classroom. If you are a K-8 district, please complete this form for two Grade-Level classrooms.
